How to Submit Your Home Blood Pressure Readings
We routinely request single or 7‑day home blood pressure readings from our patients. Submitting your readings promptly helps our clinical team review your blood pressure and plan any necessary treatment.
Our Preferred Method: Submit Readings Electronically
Where possible, we collect blood pressure readings electronically using Accurx.
How it works
- We send a secure link to your mobile phone or email address.
- You must start recording your blood pressure on the same day you receive the link.
- Enter your readings directly into the form accessed via the link.
- For 7-day blood pressure requests, please continue to use the link daily, taking your recordings every morning and evening.
- Your readings are automatically returned to us electronically.
This is the quickest and safest way for you to send us your blood pressure information.
If We Do Not Have Your Mobile Number or Email Address
If we cannot contact you electronically, we will send a letter to your home address containing:
- A blood pressure recording form, pre‑populated with your
- Name
- Date of birth
- Practice ID number
How to return the completed form
You can return your form in either of the following ways:
- Post it back to us, or
- Drop it into Haslemere Health Centre or Fernhurst Surgery in person.
Using the Home Blood Pressure Form on our Website
We also provide blank blood pressure recording forms on our website.
If you choose to use one of these forms:
- Please write your full name and date of birth clearly on the form.
- Return the completed form to Haslemere Health Centre, either
- by post, or
- in person at reception.
(Blank forms do not contain your pre-filled details, so including your personal information ensures the readings are matched to the correct medical record.)
